Parent Permission and Waiver of Liability

Raising Generals 2025-2026 School Year

Raising Generals is a non-profit ministry offering classes, tutoring, community and other activities for children. Classes are held online. The term “family” will include the adult children, minor children, and parents/guardians of minor children who have been specified on this form.

 

1. CONSENT. In consideration of the opportunity afforded my family to participate on a voluntary basis in Raising Generals, I give permission for my child(ren) to participate in Raising Generals through video conferencing as well as engaging in social interaction via Band App. I further agree that my family’s participation in Raising Generals may be terminated at any time by either the class leaders, board members from Raising Generals or myself.

 

2. RELEASE. In consideration of the opportunity afforded my family’s participation in Raising Generals, I on behalf of myself and my child(ren) hereby voluntarily fully waive, relinquish any rights, actions or causes of action resulting from personal injury or death to any member of my family or damage to my family’s property, sustained in connection with my family’s participation in Raising Generals and agree to hold Raising Generals, and its members, leaders, volunteers, successors and assigns, harmless from and against any and all claims, liabilities, rights or causes of action arising out of or as a result of my family’s participation in Raising Generals. I AGREE NOT TO SUE Raising Generals, ITS MEMBERS, TEACHERS, VOLUNTEERS, OR AFFILIATES, in connection with any injuries, or damages that occur as a result of my family’s participation in Raising Generals activities.

 

3. TECHNOLOGY. Parents agree to provide an appropriate technological environment for class participation. In general, each student will need the following:
• Windows PC or Mac computer with the most recent version of a browser (Safari, Firefox, Chrome, Internet Explorer, etc.) with the appropriate Zoom app installed
• Access to Band app for groups on web browser or app on phone. We will use this for Zoom links, reminders and discussion.
• Microphone and speakers (built-in or USB headset); A headset with a microphone is recommended for better audio quality
• Webcam
• Internet connection (broadband is best)

4. BEHAVIOR: By registering for classes with Raising Generals, parents or guardians affirm that both they and their students have read and agree to be held to the following standards:
• Discussion is encouraged and normal consideration for others is expected. Please do your best to stay on topic and refrain from interruptions while honoring others in this space. Chat facilities are intended to enhance class participation and should not be used in a manner that becomes a distraction for the student or others.
• Parents should ensure that students have a quiet space available for class attendance and participation. Please avoid consistent background noise or music playing during class.
• Students, Parents, and Auditors may not share passwords, logins, course materials, live class sessions or class archives with anyone without express written permission from Raising Generals.
• Parents, students and Raising Generals leaders are expected to follow Biblical behavior standards at all times.

 

5. KNOWING AND VOLUNTARY EXECUTION. I have carefully read this Waiver of Liability and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Raising Generals and sign it of my own free will. By signing this Waiver of Liability, I certify that I am either eighteen years of age (or older) and the parent, or legal guardian, of the named child(ren) listed in registration, or I am eighteen years of age (or older) and typing my name for the waiver for myself.
